Dynamis assists healthcare providers and the communities they serve design, finance, develop and manage Healthy Villages® which are defined as purposeful development plans designed through congregation of community leaders, members, and organizations in order to improve the Social Determinants of Health (SDOH) that the community defines as most necessary.
What Is a Healthy Village®
Healthy Villages® are communities created at the intersection of health and economic development by integrating neighborhood planning and community health. Through the coordination and integration of investments and services, Healthy Villages® improve the quality of life and enrich vitality of neighborhoods while protecting their heritages, histories, and residents. In Dynamis’ 28-year history, we have been engaged in over 90 cities and towns working in challenged neighborhoods on Healthy Village® transformations. The Healthy Villages® model is designed to enhance the traditional safety-net hospital model. The partners are engaged and selected according to investments in the drivers of health 1) Economic Stability (e.g., meaningful jobs and careers); 2) Safe Neighborhoods (e.g., housing, transportation, parks, retail services); 3) Education (from literacy to higher education and everything in-between); 4) Food security (including medically tailored meals); 5) Social Support (e.g., childcare, adult daycare); 6) Healthcare (e.g., emergency services, short-term stays including observation beds, post-acute care such as behavioral health and skilled nursing, primary care, care management, and access to quality specialty care when needed). Healthy Villages® can be designed at varying scale ranging from small, e.g., individual neighborhoods that combine primary care and care coordination with affordable housing developments), to large, e.g., central city/town plan in Reading, PA. Dynamis’ typical Healthy Village® transformation uses excess safety-net hospital space, ranging from less than 100,000 to over one million square feet/ e.g., in Milford, DE.
