The Healthy Village® at Saint Francis

Saint Francis is creating a sustainable Healthy Village that will serve the whole person and their community – emergency and acute health needs, behavioral health, long-term services and supports, education and skills development, social determinants of health (SDOH) investments, and economic revitalization. Together with partners, our aim is to create the ultimate one-stop care setting, a trusted provider that promotes synergism, diversity, and equity. Trinity Health, the not-for-profit sponsor of Saint Francis, is committed to transformation per its mission: 
 
Trinity Health Mid-Atlantic, engaged to plan the transformation.  Comprehensive interviews are critical to the process, which is consistent with the Saint Francis Community Needs Assessment, to improve the community’s health and well-being. Over 55 interviews were conducted to identify partners and interests. Dynamis architects then suggested ways to co-locate functions in the hospital and the adjacent neighborhood.
 
The initial step is creating the platform on which provider partners operate and benefit from co-location synergies. Two capital projects are budgeted in the first phase:
  • Renovations to enable Saint Francis to become partner ready:
    • New Healthy Village entrance and lobby separate from the main hospital entrance and lobby
    • Elevator tower allowing for customer flow to upper floors 
    • Senior and behavioral health ER
    • Integrated primary and behavioral health care
    • Signage and upgrades to common areas
  • Partnering with West Side Neighborhood House on nearby, supported housing. 
 
The Healthy Villages model challenges supporters to invest and partner. Many stakeholders will benefit including entities that shoulder risk for the population the HVSF serves, i.e., foundations, government, community-based organizations, behavioral health providers, housing developers, and senior service organizations who all appreciate the benefits of co-location.
 
The HVSF represents a financially sustainable solution for Saint Francis’ future. It follows where health care is headed, and the community’s interests informed by extensive interviews. It enhances critical treatment services for West Wilmington, consistent with Trinity Health’s, THMA’s and Saint Francis’s mission. And it challenges community partners to deliver on their promises.
